Key Responsibilities:
  • Maintain accurate and up-to-date records of patient encounters in the EMR
  • Assess patient needs over the phone, using evidence-based triage protocols to determine the best course of action
  • Provide high-quality care to patients, working collaboratively with multi-disciplinary teams to ensure seamless care coordination
  • Participate in ongoing education and training to maintain current knowledge and skills in telehealth nursing
