Job Summary

The Geriatric Care Manager is responsible for providing high-quality care to residents in a designated area of the facility. This role involves communicating with off-going and arriving shifts of personnel to ensure a continuum of quality care. The Geriatric Care Manager makes rounds, evaluating the physical, emotional, and social needs of residents, and ensures these needs are met through the staff assigned to work with each group of residents.

Key Responsibilities
  • Assesses assigned staff's ability to provide for each resident's needs and provides intervention as indicated.
  • Supervises the care provided by Nursing Assistants assigned to the unit, providing intervention and education as needed.
  • Makes adjustments in assignments as indicated.
  • Prepares and administers medications and treatments according to procedures.
  • Consults with physicians, receives, records, and transcribes orders as required.
  • Makes rounds with physicians when necessary.
  • Reviews resident care plans and assures these plans are followed.
  • Documents on records as per policy and procedures of the facility.
  • Participates in the evaluation of employees working under their supervision.
  • Participates in staff meetings and in-services as required.
Requirements
  • Must be a graduate of an accredited school of nursing and hold a valid nursing license in the state of employment.
  • Demonstrates leadership and managerial ability.
  • Has the ability to organize, plan, and supervise care and Nursing Assistants assigned to work a designated unit and shift.
  • Maintains effective interpersonal skills and is of good emotional, mental, and physical health, having sound judgment and high professional standards.
