About the Role:We are committed to providing high-quality, patient-centered care to individuals in their place of residence. As a Physical Therapist, you will work with patients who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes.
Key Responsibilities:- Develop and implement individualized plans of care that address patient needs and goals.
- Collaborate with patients, families, physicians, and other healthcare professionals to coordinate services and ensure seamless care.
- Provide physical therapy services, including standardized interventions and goal-oriented outcomes.
- Document patient progress and outcomes accurately and timely.
- One year of experience in acute care, rehabilitation, or home care as a Physical Therapist.
- Registered with the Board of Registration in Massachusetts.
- Strong customer service skills and ability to work with diverse populations.
- Excellent verbal, written, and electronic communication skills.
- Computer proficiency and ability to learn new software.
