Centra Healthcare Allied is seeking a skilled Physical Therapist to join their team for a contract assignment in a reputable SNF / LTC facility.

The ideal candidate will have experience in providing high-quality physical therapy services to patients with various medical conditions, including rehabilitation and treatment planning. The Physical Therapist will be responsible for assessing patients' medical conditions, functional capabilities, limitations, and restrictions, and developing a treatment plan to achieve specific goals.

The successful candidate will have a strong background in physical therapy, excellent communication skills, and the ability to work effectively in a team environment. They will also be required to maintain accurate records and provide regular updates to the facility's management team.

Requirements:

  • Graduation from an accredited physical therapy program
  • Current state license as a Registered Physical Therapist
  • Current CPR certification
  • Ability to develop a plan of care for each physical therapy patient
  • Provide skilled physical therapy services in accordance with physician orders
  • Assess patient needs, plan for, evaluate, and modify care to meet goals of physical therapy interventions
  • Collaborate with all disciplines to plan and evaluate team goals for each patient
