Job Summary

The Patient Care Coordinator plays a crucial role in assisting clinical staff and interprofessional healthcare teams in providing exceptional patient care. This position requires performing delegated activities such as patient mobility, infection control processes, and basic clerical functions under the direct supervision of clinical staff.

Key Responsibilities:
  • Patient Safety: Assist patients with practicing exercises or functional activities, transporting them to and from treatment areas, and maintaining patient safety.
  • Team Collaboration: Collaborate with the department nursing staff, physicians, and other technicians to provide seamless patient care and improve department scores for turnover/retention/employee satisfaction.
  • Infection Control: Follow infection control processes, conduct hand hygiene audits, and identify opportunities for process improvement to prevent conditions such as pressure injuries, patient falls, and hospital-acquired infections.
  • Administrative Tasks: Assist in gathering data for quality improvement projects, manage inventory of supplies, and use resources efficiently.
Requirements:
  • Educational Background: High School diploma or equivalent education.
  • Work Experience: One year Patient Care Assistant/Nursing Assistant work experience in an acute care hospital or long-term acute care hospital, or six months therapy experience, or nursing student who has completed first clinical rotation.
  • Licenses and Certifications: BLS - Basic Life Support (AHA).
Compensation:

$52,500 per year.
