As an LPN, you will play a vital role in delivering high-quality resident care, working collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.

Key Responsibilities:

  • Deliver safe and therapeutic resident care, utilizing the nursing process to ensure quality care.
  • Collaborate with the physician, family, and other healthcare team members to develop and implement individualized care plans.
  • Provide education and support to residents and their families, promoting independence and empowerment.
  • Participate in ongoing quality improvement initiatives, contributing to the development of best practices in long-term care.

Requirements:

  • Current license for Licensed Practical Nurse in the state of Wisconsin.
  • Current certification in CPR.
  • Long-term care experience preferred.
