Job Title: Home Health Care Registered Nurse

Montefiore Medical Center is seeking a skilled Registered Nurse to join our Home Health team in Fort Lee, New Jersey.


Job Summary:
As a Home Health Care Registered Nurse, you will provide high-quality patient care in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement personalized care plans to promote optimal health and well-being
  • Provide skilled nursing care, including wound care, medication management, and education
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Document patient progress and outcomes in a timely and accurate manner

Requirements:
  • Current RN licensure in New York State
  • 2 years of experience in home health care or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Knowledge of Medicare conditions of participation
