Home Health Care Registered Nurse Job Opportunity

Montefiore Medical Center is seeking a skilled Registered Nurse to join our Home Health team in Fort Lee, New Jersey.


Job Summary:
We are looking for a dedicated and compassionate Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a Home Health Care Registered Nurse, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and providing education and support to patients and their families.

Responsibilities:
• Conduct thorough assessments of patients' physical and emotional needs
• Develop and implement personalized care plans to meet patients' unique needs
• Provide education and support to patients and their families on disease management, medication, and self-care
• Collaborate with healthcare teams to ensure seamless care coordination
• Maintain accurate and up-to-date patient records

Requirements:
• Current RN licensure in New York State
• 2 years of experience in Home Health care
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of Medicare conditions of participation
