Job Description: The Clinical Case Manager will be responsible for coordinating care for clients across multiple settings, including residential treatment, partial hospitalization, intensive outpatient, and outpatient programs. This position requires strong clinical skills, excellent communication, and the ability to work effectively in a fast-paced environment.

Duties and Responsibilities:

  • Develop and implement individualized care plans for clients
  • Coordinate care across multiple settings
  • Collaborate with multidisciplinary teams to address client needs and concerns
  • Provide education and support to clients and their families regarding treatment plans and goals
  • Monitor client progress and adjust treatment plans as needed

Required Skills and Qualifications:

  • Bachelor's degree in a relevant field; LMSW preferred
  • 3+ years of experience in clinical case management or a related field
  • Strong understanding of behavioral health principles and practices
  • Excellent communication and interpersonal skills

Estimated Salary: $75,000 - $90,000 per year, based on experience
