Job Title: Private Duty Nurse LPN

Job Summary:

Aveanna Healthcare is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality, patient-centered care to children and adults in the comfort of their own homes. As a Private Duty Nurse LPN, you will work closely with our patients and their families to develop and implement personalized care plans that meet their unique needs and goals.

Responsibilities:

  • Implement physician-prescribed plans of care and document patient progress in a timely and accurate manner
  • Provide one-on-one patient care in a home environment, including administration of medications, treatments, and therapies
  • Conduct patient assessments and coordinate care with other healthcare professionals as needed
  • Health promotion, teaching, and training of family members to ensure optimal patient outcomes
  • Collaborate with the care team to ensure seamless transitions of care and address any concerns or issues that may arise

Requirements:

  • Active, unencumbered LPN license in the state of practice
  • Current CPR certification from the American Red Cross or American Heart Association
  • 6 months of hands-on nursing experience, preferably in home health care
  • Ability to work independently and as part of a team to provide high-quality patient care
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
