About the Role

We are seeking a skilled Hospice Registered Nurse to join our team at Careline Health Group. As a Hospice Nurse, you will play a vital role in providing high-quality care to terminally ill patients and their families in a home or facility setting.

Key Responsibilities
  • Conduct initial evaluations and re-evaluate patient nursing needs during each visit.
  • Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating patient care plans.
  • Complete assessments specific to the provision of Hospice care, including identification of home safety risks and availability of support systems.
  • Provide support at the time of death and perform bereavement assessments if requested.
  • Communicate effectively with physicians, patients, families, facilities, and other organization personnel.
Requirements
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current State License as a Registered Nurse.
  • Minimum one (1) year experience as a nurse in a clinical care setting.
  • Nursing skills as defined by accepted nursing standards.
  • Ability to assess and respond to patient needs in varied settings.
Preferred Qualifications
  • BSN preferred.
  • Experience as a nurse in a hospice or home health environment.
  • Familiarity with Home Care Home Base (HCHB).
