Home Care Registered Nurse Opportunity

Main Line Health, Inc is seeking a skilled Home Care Registered Nurse to join our team. As a Home Care Registered Nurse, you will provide coordinated, comprehensive nursing care to patients in their homes, utilizing your critical thinking and clinical judgment skills to address their physical, psychosocial, and spiritual needs.

Responsibilities:
  • Provide high-quality, patient-centered care in the home setting, addressing the unique needs of each patient and family.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans, ensuring seamless transitions and optimal patient outcomes.
  • Utilize evidence-based practices and clinical guidelines to deliver care that is safe, effective, and efficient.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and shared goals.
Requirements:
  • Current license to practice nursing in PA.
  • Two years of recent med-surg experience in an acute or long-term care setting, rehabilitation, home care, or hospice setting.
  • Certification in specialty area of nursing preferred.
