About the Role

Key Responsibilities
  • Provide coordinated, comprehensive nursing care to patients in their homes, addressing their physical, psychosocial, and spiritual needs.
  • Use critical thinking, analysis, and clinical judgment skills to deliver high-quality care and improve patient outcomes.
  • Develop and implement personalized care plans, working collaboratively with patients, families, and healthcare teams.
  • Conduct thorough assessments, identify patient needs, and develop strategies to address them.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
Requirements
  • Current license to practice nursing in Pennsylvania.
  • Valid driver's license and automobile insurance.
  • Current CPR certification.
  • Two years of recent med-surg experience in an acute or long-term care setting, rehabilitation, home care, or hospice setting.
  • RN Diploma, ASN required; BSN preferred.
