Job Summary

We are seeking a highly skilled Advanced Illness Management Social Worker to join our team. As a key member of our care management team, you will work closely with patients, their families, and healthcare professionals to provide comprehensive social work services.

Key Responsibilities

  • Develop and implement plans of care to address the social and emotional needs of patients and their families
  • Assess crisis situations and provide appropriate resources and referrals
  • Provide individual and family interventions to support patients and their loved ones
  • Collaborate with healthcare professionals to ensure seamless care coordination

Requirements

  • Masters of Social Work from an accredited institution
  • Able to attain multistate licensing as a LMSW or LCSW, with a focus on Pennsylvania licensure
  • 2 years of prior social work experience in a healthcare setting, preferably in palliative care, advanced illness management, or hospice
  • Strong understanding of palliative care and hospice philosophy, including issues related to death and dying
  • Knowledge of the impact of terminal diagnosis and anticipatory grief on psychosocial functioning, family dynamics, and community relations
  • Ability to establish and maintain therapeutic relationships with patients, families, and colleagues
