Cardiovascular Care Coordinator

RN Patient Navigators at University Hospitals play a critical role in guiding patients through their healthcare journey. As a Care Coordinator, you will be responsible for ensuring seamless transitions between various care settings, from pre-op to post-op, and facilitating interdisciplinary collaboration to achieve optimal patient outcomes.
Responsibilities:
- Monitor and guide patients through the complete experience, performing clinical screening, coordinating evaluation, and scheduling appointments.
- Facilitate collaboration among healthcare professionals to ensure effective care delivery.
- Document delegated portions of care and interventions in medical records.
- Support patient education initiatives and serve as a resource to patients and families.
Requirements:
- Bachelor's Degree in Nursing (BSN) or Associate's Degree in Nursing (ADN).
- 5+ years of clinical medicine experience or 3+ years of cardiology experience.
- Ambulatory nursing experience is preferred.
