Job Title: Coordinator, Wellness Program Mobile Services

We are seeking a highly motivated and organized Coordinator to join our team at Project Open Hand. As a key member of our Wellness Program, you will play a vital role in expanding our mobile services to increase access to our programs for clients in their communities.

Job Summary:

The Coordinator, Wellness Program Mobile Services will be responsible for managing all operational aspects of our Mobile Services program, including coordinating with our Grocery Center Operations and Distribution departments to ensure seamless execution of our services. This role will also involve providing case management to promote client engagement and maintaining accurate client files. Additionally, you will establish and maintain policies and procedures for the Mobile Services Program, document and analyze trends related to quality assurance and quality improvement, and identify opportunities for program growth.

Key Responsibilities:

  • Manage all operational aspects of POH's Mobile Services program, including coordinating with Grocery Center Operations and Distribution departments
  • Provide case management to promote client engagement and maintain accurate client files
  • Establish and maintain policies and procedures for the Mobile Services Program
  • Document and analyze trends related to quality assurance and quality improvement
  • Identify opportunities for program growth and develop strategies to achieve them

Requirements:

  • Three years' experience providing direct services in a Social Service environment
  • Bachelor's Degree in a Human Service field or equivalent experience
  • Proficient in MS Office and database management
  • Strong organization and communication skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks

Preferred Qualifications:

  • Experience working in a nonprofit client services/community engagement-based setting
  • Mental Health or Social Work Experience
  • ServSafe Certification with demonstrated knowledge of safe food handling practices
  • Experience supervising staff and/or volunteers
