Nurse Practitioner

UNM Medical Group, Inc. is seeking a highly skilled Nurse Practitioner to join our Behavioral Health Clinic team. As a key member of our interdisciplinary team, you will provide accessible behavioral health care to the Sandoval County community, while also coordinating care with integrated behavioral providers at the neighboring Sandoval Regional Medical Center and nearby Grande Primary Care Clinic.
Responsibilities- Evaluate and manage behavioral health care plans for patients (aged 18 and up) with psychiatric illnesses, substance use disorders, and problems related to chronic health conditions.
- Work collaboratively with other members of the inter-professional healthcare team, including psychiatry, psychology, clinical counseling/social work, and coordination of care with other health care providers and community resources.
- Provide accurate documentation of patient care, document orders, directives, procedures, and patient responses in the medical record and communicate them to other members of the healthcare team.
- Act as a role model through conduct, appearance, communication, mutual respect, leadership, ethical decision-making, critical thinking, and problem-solving skills.
The Behavioral Health Clinic has an interdisciplinary team providing accessible behavioral health care to the Sandoval County community, while also coordinating care with integrated behavioral providers at the neighboring Sandoval Regional Medical Center and nearby Grande Primary Care Clinic. Team members work collaboratively across behavioral health and related disciplines, including psychiatry, psychology, nursing, primary care, clinical social work, counseling, and peer support.
