Job Summary

Key Responsibilities

  • Build trust between patients and medical practitioners

  • Teach, counsel, and monitor patients on health-related issues
Requirements
  • Associates or Bachelor's Degree in Nursing and three years' work experience related to chronic disease management
  • Practical Nursing Degree and five years' experience related to chronic disease management
  • Licensure/Certification: Must be licensed as a Practical Nurse or Registered Nurse by the South Carolina Board of Nursing
  • BLS CPR certification
Preferred Qualifications
  • Familiarity with local community resources for patients with chronic disease
  • Knowledge of patient teaching, health promotion, and disease prevention methods
  • Understanding of PCMH concepts
