Job Summary:

Medical Director Services PC is seeking an experienced Nurse Practitioner to provide primary care services to residents in a skilled nursing facility in Philadelphia, PA. The ideal candidate will have a strong background in geriatric care and be able to work collaboratively with the medical attending and nursing staff to provide high-quality care.

Responsibilities:

  • Assess and manage the health needs of residents, including performing physical examinations and obtaining medical histories
  • Develop and implement individualized care plans to meet the unique needs of each resident
  • Collaborate with the medical attending and nursing staff to ensure seamless care transitions and effective communication
  • Participate in interdisciplinary team meetings to discuss resident care and develop strategies for improving outcomes
  • Stay up-to-date with the latest clinical guidelines and best practices in geriatric care

Requirements:

  • Board certification as a Nurse Practitioner
  • Experience working in a skilled nursing facility or similar setting
  • Strong knowledge of geriatric care and ability to work with older adults
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a team
