About the Role

As a skilled Care Coordinator RN Specialist, you will play a vital role in coordinating patient care along the full continuum. Your expertise will ensure optimal clinical, financial, functional, and health status improvements in our target population.

The ideal candidate will possess strong critical-thinking skills, able to apply clinical guidelines, policies, and regulatory requirements to determine the appropriate level of care. You will monitor patients' clinical status and needs, anticipating and preparing for transitions of care.

Your goal will be to promote patient engagement and participation in their care at their highest attainable level, while managing patients in the least restrictive environment possible.
