Care Specialist

FACETS, a non-profit organization, is seeking a Care Specialist to provide supportive services to chronically homeless individuals in a group home setting.
Job Summary
The successful candidate will provide direct supervision and assistance to residents, assist with developing individual service plans, and collaborate with staff members to ensure resident needs are met. A valid driver's license, reliable transportation, and basic knowledge of facility cleaning and repairs are required.

Responsibilities
- Provide direct supervision and assistance to residents
- Assist case manager with developing, implementing, and monitoring individual service plans
- Collaborate with other staff members to ensure residents' needs are met
- Monitor and report maintenance, health, fire, and safety issues
- Ability to learn HMIS documentation
- Represent FACETS in the community, aligning with Agency Mission, Vision, and Code of Ethics
- Perform other duties as assigned
Requirements
- Bachelor's Degree preferred or equivalent working experience
- Two years of professional social work/case management experience with homeless populations
- Knowledge of current social service, homeless, and housing issues and methods/approaches to address them
- Candidate must possess a valid driver's license, reliable transportation, good driving record, and personal car insurance
- Must provide proof of COVID vaccination
- Candidate will be required to pass criminal background and Child Protective Services background checks
- CPR Certification Preferred
Benefits
- $17.00 per hour
