The Registered Nurse in Care Management is responsible for the comprehensive planning, coordination, and delivery of nursing care and health education to residents in a skilled nursing facility.
Key Responsibilities- Supervise the nursing care provided by LPNs, CNAs/STNAs, and other team members.
- Conduct health assessments of residents and communicate any changes in their condition to the physician.
- Develop individualized care plans based on assessments and implement appropriate nursing interventions.
- Contribute to resident assessments (MDS/CAA's) and assist in the formulation of care plans.
Preferred candidates will have 1-3 years of experience in a long-term care environment.
