Clinical Diabetes Educator

The Clinical Diabetes Educator oversees a program of education for clients of all ages with chronic disease, including diabetes, cardiovascular, and renal dysfunction. This process involves a health and education assessment, development, implementation, and evaluation of an education plan, considering psychosocial, economic, and language barriers. The Educator collaborates with a professional team in a classroom or individual outpatient settings.
This position is responsible for all aspects of the accredited Diabetes Self-Management Education and Training (DSME/T) program. The Educator incorporates professional development and community outreach as an integral part of professional practice.
Responsibilities:
- Develop and implement education plans for clients with chronic disease.
- Collaborate with healthcare teams to provide education and support.
- Conduct health and education assessments to identify client needs.
- Evaluate the effectiveness of education programs and make recommendations for improvement.
Requirements:
- Master's degree in nursing, dietetics, or a related field.
- Current certification as a Registered Nurse or Registered Dietician.
- Experience in diabetes education and program management.
- Knowledge of current pediatric and adult learning theory.
