Nurse Home Health Specialist

BAYADA Home Health Care is seeking a skilled Registered Nurse (RN) to provide high-quality care to clients in their homes. As a Home Health RN, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.
Job Summary:We are looking for a compassionate and dedicated Registered Nurse to join our team. As a Home Health RN, you will be responsible for providing skilled nursing care to clients in their homes, including administering medications, wound care, and treatments. You will also be responsible for monitoring clients' conditions, reporting changes to our Clinical or Client Services Manager, and following up with doctors' orders.

- Follow up with doctors' orders and ensure that clients receive the care they need.
- Develop and implement individualized care plans to meet the unique needs of each client.
- Collaborate with other healthcare professionals to ensure that clients receive comprehensive care.
- A current license as a Registered Nurse in Vermont.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience is strongly preferred, but not required.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
