POSITION TITLE: RESIDENTIAL SUPPORT SPECIALIST

Momentum WORK, Inc. is in search of a Residential Support Specialist II in Santa Maria, CA, dedicated to enhancing the quality of life and fostering meaningful connections with the individuals we support. This role emphasizes promoting autonomy, personal development, and ensuring a safe and nurturing home environment for those in our Residential Services program.

KEY RESPONSIBILITIES:

  • Deliver personalized coaching in essential daily living skills.
  • Provide guidance and assistance in utilizing community resources.
  • Keep precise and up-to-date records in ID notes and communication logs pertinent to Individual Service Plans.
  • Support individuals in adhering to their daily activity schedules, which includes meal preparation, maintaining a clean and organized living space, managing household tasks, budgeting, and transportation as needed.
  • Oversee medication administration.
  • Assist participants with all aspects of personal care, including attending medical and dental appointments.
  • Encourage individuals to participate in social activities to enhance their social skills.
  • Educate program participants on accessing public transportation and assist them in obtaining benefits such as Social Security, SSI, IHSS, and Section 8 housing.
  • Support individuals with both weight-bearing and non-weight-bearing transfers.

REQUIREMENTS:

  • High school diploma or equivalent.
  • Successful completion of a clear live scan background check and physical examination.
  • Reliable transportation (valid CA driver's license and vehicle insurance required).
