About the Role

We are seeking a highly skilled Clinical Effectiveness Coordinator to join our team at Foundations for Living. As a key member of our quality improvement team, you will play a critical role in analyzing and reporting on key clinical performance measures, identifying areas for improvement, and developing strategies to enhance patient care and outcomes.

Responsibilities
  • Analyze and report on key clinical performance measures, including stroke, sepsis, and other quality indicators
  • Identify and access relevant data sources, aggregate and manipulate data as needed, and prepare graphs and tables from data
  • Ensure the integrity and accuracy of data and reported measures, and submit information to regulatory agencies as requested
  • Develop and maintain clinical registries and associated datasets, and support the analysis and reporting needs of organization-wide quality improvement projects
  • Collect and submit accurate, complete, and timely data for specific registries, and oversee daily management of retrospective chart reviews
Requirements
  • Associate degree from an accredited College or University in Nursing required
  • Bachelor's degree from an accredited College or University preferred
  • Three to five years of clinical experience as a Registered Nurse (RN) in an acute care setting required
  • Previous experience in Quality Management or Performance Improvement preferred
  • 3 years of progressive clinical experience in neurological nursing and RN Licensure, ACLS, and BLS
Preferred Qualifications
  • Certified Professional in Healthcare Quality (CPHQ) certification
  • Previous experience in case review, quality management, chart audits, and/or implementing actions to ensure regulatory compliance
  • Understanding of the goals of the Center of Excellence (COE) programs (Stroke & Sepsis)
