We are seeking a highly skilled Medical Care Coordinator to join our team. This is an excellent opportunity for a registered nurse to utilize their skills and experience in a fast-paced environment.

About the Role
This position will be responsible for coordinating patient care across the healthcare continuum. The ideal candidate will have strong communication and interpersonal skills, with the ability to work effectively in a virtual setting.

Key Responsibilities:
• Telephonically triage patients and navigate care based upon evidence-based protocols.
• Provide patient education across the healthcare continuum.
• Engage patients in healthy behaviors through telemonitoring activities.
• Demonstrate effective communication skills and work well with others, including team members, patients, caregivers, referral sources, physicians, and facility staff.

Requirements:
• 3 years or more of RN experience in Emergency or Acute Care setting.
• Graduate of an accredited school of nursing.
• Strong verbal, written, and interpersonal communication skills.
• Nursing skills as defined by generally accepted standards of practice.
