Job Summary

Alternate Solutions Health Network LLC is seeking a skilled Physical Therapist to join our team in Cuyahoga Falls, Ohio. As a Home Health Physical Therapist, you will provide high-quality care to patients in their homes, working closely with our interdisciplinary team to achieve positive outcomes.

Key Responsibilities
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and other relevant staff members.
  • Counsel patients and their families on disease management and prevention, and instruct them on proper use of equipment such as wheelchairs, braces, and other prosthetic/orthotic devices.
  • Supervise and oversee therapy services provided by Physical Therapist Assistants and nurse assistants, as needed.
  • Complete accurate documentation and remain compliant with all legal rules and regulations.
Requirements
  • Current licensure as a Physical Therapist in the state of Ohio.
  • Experience in home health care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
