Job Title: Behavioral Health Care Manager, Adult

Mental Health Cooperative is seeking a highly skilled Behavioral Health Care Manager to join our team. As a Behavioral Health Care Manager, you will be responsible for providing comprehensive care management services to consumers with severe and persistent mental illness (SPMI) and their supports.

Responsibilities:
  • Conduct continual assessments of consumers' strengths, needs, supports, and goals.
  • Meet with consumers and their supports in the community to provide support and guidance.
  • Collaborate with consumers and their supports to develop comprehensive and individualized care plans, crisis plans, and other necessary documents.
  • Assist consumers with accessing internal and community resources to ensure they receive the necessary supports.
  • Provide education to consumers and their supports on proper usage of emergency services and other healthcare resources.
  • Ensure consumers' daily psychiatric, medical, and environmental needs are planned for and coordinated.
  • Meet productivity standards and participate in regular supervision and team meetings.
Requirements:
  • Bachelor's degree in a health-related field, such as counseling, psychology, or social work.
  • Valid Tennessee driver's license and acceptable motor vehicle report.
  • Acceptable criminal background investigation and personal automobile insurance.
  • Cell phone and transportation that seats 4 people.
