Certified Home Health Care Professional

CVHCare Home Health is seeking a skilled Certified Home Health Aide to join our team. As a Certified Home Health Aide, you will work in collaboration with licensed clinicians to provide patients with skilled services and assistance with daily activities. Your role will involve checking vital signs, assisting with exercises, and providing support with daily routines such as bathing, dressing, and feeding. You will also be responsible for maintaining accurate documentation of patient care and communicating effectively with the healthcare team. We are looking for a compassionate and detail-oriented individual with a strong understanding of clinical terminology and experience working in a home health setting. **About the Position:**
Our Certified Home Health Aides work in a fast-paced and dynamic environment, providing high-quality care to patients in their homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
**Responsibilities:**
* Provide skilled services and assistance with daily activities to patients in their homes
* Check vital signs, assist with exercises, and provide support with daily routines
* Maintain accurate documentation of patient care and communicate effectively with the healthcare team
* Collaborate with licensed clinicians to develop and implement patient care plans
* Provide education and support to patients and their families on health-related topics
**Requirements:**
* Minimum one year experience working as a Certified Home Health Aide
* Minimum one year checking vitals and reporting/documenting on clinical charts
* Home Health Aide Training
* California Home Health Aide Certification and CPR Certification
* General knowledge and understanding of clinical verbiage
* Strong attention to detail for use in chart documentation
* Excellent time management/calendaring skills and ability to deliver documentation timely
* Strong experience using computers and computer systems
* Experience using Microsoft Suite (Word, Excel, Outlook), Gmail Suite or other systems
* Excellent written and verbal communication
**Schedule:**
* Part-time
* 24 = 18 RVEs p/week or Per Diem (6 visits p/week)
**Locations:**
* Area #4 covers the following cities - Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafeyette, Orinda, Pleasant Hill, Walnut Creek
* AND/OR
* Area #5 covers the following cities - Pittsburg, Discovery Bay, Antioch, Brentwood, Oakley, Byron, Bethel Island, Bay Point
