We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team at Matrix Medical Network.
The ideal candidate will have a strong background in healthcare and be able to conduct comprehensive health assessments in various settings, including home visits. They will work collaboratively with physicians, case managers, and other healthcare professionals to provide high-quality patient care and support.
Key Responsibilities- Conduct thorough health assessments of patients in their homes, identifying potential health risks and developing personalized plans to address them
- Collaborate with healthcare teams to ensure seamless care coordination and effective communication
- Develop and implement treatment plans tailored to individual patient needs
- Maintain accurate and detailed records of patient interactions and care provided
- Master's degree in Nursing or Physician Assistant studies required
- Current RN and NP/PA licensure in the state of practice, with prescription authority or ability to obtain
- Board certification in a relevant specialty
- At least one year of experience as a Nurse Practitioner or Physician Assistant
- Excellent communication and interpersonal skills
- Ability to travel and work flexible hours
This is a PRN opportunity with competitive compensation and mileage reimbursement for completed assessments. Additionally, we offer opportunities for professional growth and development within our organization.
Estimated Salary: $110.00 per completed assessment (25 hours or more/month) + flexible scheduling
