About the Position

As a skilled Home Health Risk Assessment Nurse, you will play a vital role in improving the quality of care for older adults and other at-risk individuals. Our comprehensive health assessment provides unparalleled insight into an individual's overall health and well-being, allowing us to identify chronic conditions that may otherwise go undiagnosed.

In this dynamic position, you will have the opportunity to leverage your expertise and compassion to make a direct impact on the lives of others. You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel to provide patient-centered care.

Responsibilities:

  • Conduct adult/geriatric assessments, medical history reviews, diagnosis, treatment planning, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols
  • Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up support
  • BUILD AND MAINTAIN RELATIONSHIPS WITH A NATIONWIDE NETWORK OF ADVANCED PRACTICE PROVIDERS
  • Deliver services in various settings, including home visits, skilled facility visits, and telehealth visits

We offer a competitive compensation package, including $90.00 per completed assessment, flexible scheduling, mileage reimbursement, and no-show payment incentives.

Qualifications:

  • Master's degree required OR commensurate experience and satisfactory completion of NP licensure
  • Current RN and NP licensure in state of practice, including prescription authority or ability to obtain prescriptive authority
  • Board certification by the AANP, ANCC, or AACN in a Matrix-approved specialty
  • 1 year of experience as a Nurse Practitioner
  • Strong computer skills and familiarity with Employee Health/Medical Record software
  • Excellent verbal and written communication skills with patients, clients, and colleagues
