Job Title: Family Advocate

VNS Health is seeking a compassionate and dedicated Family Advocate to join our team. As a Family Advocate, you will play a vital role in supporting children and families in accessing life-changing resources and services.

Key Responsibilities:
  • Assist clients in navigating complex systems and accessing entitlements, including Medicaid, Public Assistance, and SSI.
  • Provide emotional support and guidance to clients and their families, helping them to cope with challenging situations.
  • Develop and maintain relationships with community resources and service providers to ensure seamless referrals and support.
  • Collaborate with interdisciplinary teams to discuss care needs and identify solutions to support clients.
  • Attend planning meetings with service providers to coordinate service plans.
Requirements:
  • Bachelor's Degree in social work, health and human services, or a related field.
  • Minimum of one year of experience in a mental health or social service setting with emphasis on family support services and/or family psycho-education.
  • Lived experience as a caregiver for children with behavioral health needs in accordance with the New York State Office of Mental Health and the New York City Department of Health and Mental Hygiene.
