About the Role

We are seeking a skilled Service Coordinator to join our team at Family Service League Inc. as part of our Children's Care Coordination Program in Bay Shore, NY.

The ideal candidate will have a passion for supporting children and young adults with mental illness and/or chronic medical conditions in achieving their goals. As a Service Coordinator, you will be responsible for developing individualized plans to support clients, conducting home visits and assessments, providing linkages, and assisting with navigating service systems.

Key Responsibilities:

  • Develop strengths assessments with clients.
  • Develop crisis/safety planning in collaboration with mental health providers.
  • Create individualized plans with clients in areas such as health, education, mental health, vocations, socialization, benefits, legal, and life skills.
  • Meet minimum of two times a month to support clients in goal achievements.
  • Assist in coordination and linkage of services with providers, benefit systems, and community resources.
  • Help clients navigate service systems to access necessary support.
