MDS Coordinator

Devonshire Care Center is seeking a skilled MDS Coordinator to participate in the Resident Assessment Process and contribute to the development of individualized resident care plans.

• Direct the facility interdisciplinary RAI process while following HHC guidelines for completion, computer encoding, and electronic State transmission of the MDS.
• Complete nursing assigned MDS items, RAPS, and Care Plans as designated by HHC.
• Participate in resident Care Plan Meetings.
• Train new members of the IDT on clinical content and electronic completion of the applicable MDS items, RAPS, and writing of individualized resident Care Plans.
• Participate in the daily Morning Meeting per HHC guidelines.
• Print Nursing Home Quality Indicator Reports from the State database every month and as requested by the facility Director of Nursing.
• Maintain resident confidentiality and privacy; treat residents with kindness, dignity, and respect; know and comply with and ensure that all nursing services personnel know and comply with Resident's Rights rules.
• Physical and Sensory Requirements: Ability to communicate with residents and personnel and ability to remain calm under stress.
• Maintains on-site regular attendance as scheduled.
• Complies with applicable legal requirements, standards, policies, and procedures including but not limited to those within the Compliance Process, Code of Conduct, and HIPAA.

• Ability to communicate effectively with residents, personnel, and other stakeholders.
• Strong analytical and problem-solving skills.
• Ability to work independently and as part of a team.
• Excellent organizational and time management skills.
• Ability to maintain confidentiality and handle sensitive information.
