Position Overview:

Devonshire Care Center is seeking a dedicated MDS Coordinator (LVN or RN) to enhance our skilled nursing team.


Compensation:

Competitive pay ranging from $35 to $47 per hour.


Role Summary:

The MDS Coordinator plays a crucial role in the Resident Assessment Process by completing essential components of the Minimum Data Set (MDS) and the associated Care Area Assessments (CAAs). This position also oversees the Resident Assessment Protocol Summaries (RAPS).


Key Responsibilities:
  • Lead the interdisciplinary RAI process, ensuring compliance with guidelines for MDS completion and electronic submission.
  • Complete assigned MDS items, RAPS, and Care Plans as directed.
  • Engage in resident Care Plan Meetings to develop tailored care strategies.
  • Provide training for new interdisciplinary team members on MDS items and Care Plan documentation.
  • Participate in daily meetings to discuss resident care and quality indicators.
  • Generate and analyze Nursing Home Quality Indicator Reports monthly.
  • Ensure resident confidentiality and uphold residents' rights with dignity and respect.
  • Maintain regular attendance and comply with all legal and ethical standards.
  • Facilitate compliance training and ensure staff awareness of policies and procedures.
  • Participate in quality improvement initiatives and audits as necessary.
