Compassionate Hospice Caregiver

Position Overview:
The Hospice Aide is responsible for delivering direct patient care services within a defined scope of practice. This role supports patients and nursing staff by executing nursing interventions as outlined by the Registered Nurse. Services are provided in the comfort of the patients' homes as directed.
Key Responsibilities:
- Engage in patient care conferences and collaborative meetings.
- Complete a minimum of 12 hours of approved continuing education annually.
- Treat patients, physicians, and colleagues with kindness, courtesy, and respect.
- Assist with personal hygiene tasks, including bathing, skin care, oral hygiene, and changing bed linens.
- Support patients in using bathroom facilities and assist with mobility and transfer activities.
- Facilitate patient movement in and out of bed, utilizing mechanical lifting equipment when necessary.
- Assist physical therapists with non-skilled rehabilitation treatments essential for patient recovery.
- Help patients with ambulation, including the use of walkers and wheelchairs.
- Contribute to meal preparation and feeding when required.
- Perform essential household tasks that support the patient's health care at home and help prevent institutionalization.
- Assist with prescribed exercises as directed by healthcare professionals.
- Measure and document fluid intake and output, and record vital signs such as blood pressure, pulse, and respiration rates.
- Complete daily worksheets summarizing care provided to each patient.
- Report any changes in the patient's condition, behavior, or appearance to the nurse supervisor promptly.
- Maintain organized records of individual Aide assignments for each patient.
- Submit Visit Reports as directed.
- Uphold a professional appearance in accordance with agency uniform policies.
- Provide appropriate notice for absences or tardiness without misuse.
- Adhere to infection control and safety protocols as established by the home health agency.
- Maintain focus on work responsibilities without personal distractions.
- Participate willingly in special projects or overtime as requested.
The role demands good physical health to meet the active demands of the position, including extensive walking, driving, sitting, standing, and lifting. The ability to assist patients weighing 100 lbs. or more is essential.
Qualifications:
Applicants should have at least one year of experience in hospice or home health care, along with a solid understanding of the fundamental concepts of hospice or home health care and their role within a patient care team. Candidates must hold the appropriate certification and have completed the required curriculum/hours, with competency evaluated in accordance with Federal and State regulations. LPNs with CNA certification will also be considered.
Emotional and mental maturity is necessary for fostering and maintaining positive working relationships with patients, families, and other healthcare personnel.
