Job Summary

We are seeking a highly skilled Physical Therapist to join our team at Conemaugh Home Health, a part of LHC Group. As a Physical Therapist, you will be responsible for assessing and evaluating patient care needs, developing treatment plans, and providing interventions to improve patient well-being.

Key Responsibilities
  • Evaluate patient functional status, physical therapy needs, and medical conditions
  • Develop and implement treatment plans in accordance with state laws and evidence-based practices
  • Provide patient education and inform participating members of the healthcare team about the plan of care
  • Visit patients within the service area and submit progress notes as required
Requirements
  • Current Physical Therapy licensure in the state of PA
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
