Job Summary

We are seeking a skilled Physical Therapist to join our team at Conemaugh Home Health, a part of LHC Group, the preferred post-acute care partner for hospitals, physicians, and families nationwide.

As a Physical Therapist with our team, you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their goals and improve their overall well-being.

Key Responsibilities
  • Evaluate patient care needs and develop personalized treatment plans to improve functional status, endurance, and overall well-being.
  • Provide physical therapy interventions, including exercises, stretches, and other therapies, to help patients achieve their goals.
  • Monitor patient progress and adjust treatment plans as needed to ensure optimal outcomes.
  • Collaborate with other healthcare professionals, including physicians, nurses, and home health aides, to ensure comprehensive care and support.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with treatment plans.
Requirements
  • Current Physical Therapy licensure in the state of PA
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
