We are seeking a highly skilled and dedicated Registered Nurse to join our team as a Holistic Primary Care Nurse Specialist. This role will provide comprehensive nursing care to patients across the lifespan, focusing on personalized, whole-hearted care.

As a member of our Clinical Care team, you will be responsible for monitoring patient health, providing treatments, administering medications, and educating patients on health maintenance and disease prevention. You will work closely with medical staff to ensure quality care is provided to our patients.

The ideal candidate will have three years' experience as a RN in a physician's office, family practice, or similar medical setting. You will possess strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.

Key Responsibilities:

  • Provide comprehensive nursing care to patients across the lifespan
  • Monitor patient health, provide treatments, administer medications, and educate patients on health maintenance and disease prevention
  • Collaborate with medical staff to ensure quality care is provided to patients
  • Triage, assess, plan, schedule, coordinate, evaluate, and delegate the delivery of patient-centered nursing care
  • Identify at-risk patients, develop care plans, and coordinate care with patient involvement

Requirements:

  • WA State RN licensure
  • Three years' experience as a RN in a physician's office, family practice, or similar medical setting
  • BLS required, current AHA Healthcare Provider training (BCLS)
  • Knowledge of Medicare documentation, billing, and reporting requirements associated with Annual Wellness Visits (AWV), Chronic Care Management (CCM), and Transitional Care Management (TCM) services
