About the Role

We're seeking a dedicated Advanced Primary Care Provider to join our team. As an autonomous practitioner, you'll focus on relationship-based medicine, offering holistic care that includes prevention, chronic disease management, and health education. With access to top-notch resources like UpToDate and RubiconMD, you'll be equipped to provide high-quality patient care.

Key Responsibilities
  • Patient-centered approach to care, prioritizing prevention and wellness
  • Chronic disease management and health education
  • Urgent visits and routine wellness care
Requirements
  • Active license & current ANCC or AANP board certification required
  • Federal DEA number preferred; may be required for full prescribing ability
  • CPR/BLS certification required at time of start date
