About the Role

We are seeking a compassionate and dedicated Hospice Care Specialist to join our team at AccentCare. As a Hospice Aide, you will play a vital role in providing high-quality patient care and support to individuals and their families in need.

Key Responsibilities
  • Provide direct patient care and support to individuals in need, including assistance with daily living activities, medication management, and emotional support.
  • Work collaboratively with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and document findings in a timely and accurate manner.
  • Develop and maintain strong relationships with patients, families, and healthcare providers to ensure seamless care coordination and communication.
  • Participate in ongoing education and training to stay up-to-date on the latest hospice care practices and regulations.
Requirements
  • High School Diploma or equivalent required.
  • Licensed to practice as a Certified Nursing Assistant (CNA) in the state of agency operation.
  • Valid CPR certification preferred.
  • Must be a licensed driver who can travel to all business locations.
  • Meet the regulations and requirements of the state(s) in which the program provides service.
