Key Responsibilities- Provide direct patient care and support to individuals in need, including assistance with daily living activities, medication management, and emotional support.
- Work collaboratively with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct thorough assessments of patients' physical, emotional, and social needs, and document findings in a timely and accurate manner.
- Develop and maintain strong relationships with patients, families, and healthcare providers to ensure seamless care coordination and communication.
- Participate in ongoing education and training to stay up-to-date on the latest hospice care practices and regulations.
- High School Diploma or equivalent required.
- Licensed to practice as a Certified Nursing Assistant (CNA) in the state of agency operation.
- Valid CPR certification preferred.
- Must be a licensed driver who can travel to all business locations.
- Meet the regulations and requirements of the state(s) in which the program provides service.
- Competitive hourly rate of $21.00 per hour, based on years of experience.
- Monday - Friday schedule, 8:00am - 5:00pm.
