Role Overview:

Under the guidance of the Launch Medical Director, the Travel Nurse Provider collaborates closely with WelbeHealth Physicians and the IDT to deliver assessments, develop care plans, and provide primary care services to our participants.

Key Responsibilities:

  • Conduct thorough in-person assessments, including initial, semi-annual, and unscheduled evaluations, along with continuous medical oversight for participants.
  • Work alongside physicians and the IDT to formulate individualized care plans for each participant.
  • Assist in discharge planning from both acute and long-term care settings in collaboration with the medical team.
  • Maintain regular visits to nursing homes to ensure ongoing care for participants as needed.
  • Coordinate effectively with WelbeHealth physicians, community-based PACE physicians, and the IDT to ensure seamless care delivery.
  • Travel to various centers as required based on participant needs.
  • Review outcomes and utilization data, including hospitalizations, to identify best practices and areas for policy enhancement.
  • Participate in a rotating schedule for night and weekend on-call duties.

Qualifications:

  • Graduation from an accredited nursing program.
  • Certification as a Nurse Practitioner or Physician Assistant with prescriptive authority.
  • Active RN state license.
  • Current Drug Enforcement Administration (DEA) registration.
  • Eligibility for enrollment to provide services to Medicare and Medi-Cal participants.
  • A minimum of two years of experience working with the senior demographic, with current knowledge of chronic care and geriatric best practices.
  • Reliable transportation is essential.

Compensation:

The salary range for this position is $145,000 - $174,478 per year, plus bonuses and equity. WelbeHealth offers a competitive total rewards package, including healthcare coverage and a variety of other benefits, with actual pay adjusted based on experience and qualifications.
