WelbeHealth PACE (Program of All-Inclusive Care for the Elderly) empowers seniors to maintain their independence in their homes and communities. We utilize a collaborative interdisciplinary team (IDT) model to assess and manage participant care, which is integral to our values and culture.

Role Overview

Under the guidance of the Launch Medical Director, the Travel Nurse Provider collaborates with WelbeHealth Physicians and the IDT to deliver comprehensive assessments, care planning, and ongoing medical education to our participants.

Key Responsibilities:

  • Conduct thorough in-person assessments, including initial evaluations, semi-annual reviews, and urgent assessments, ensuring continuous medical coverage for participants.
  • Work closely with physicians and the IDT to formulate individualized care plans for each participant.
  • Assist in discharge planning from both acute and long-term care settings.
  • Support the ongoing care of participants residing in nursing facilities through regular visits as required.
  • Collaborate with WelbeHealth physicians, community-based PACE physicians, and the IDT to ensure effective care coordination.
  • Travel to various centers as needed to meet participant requirements.
  • Review outcomes and utilization data, including hospitalizations and specialist usage, to identify best practices and enhance policies.
  • Participate in a rotating schedule for night and weekend on-call duties.

Qualifications:

  • Graduation from an accredited nursing program.
  • Certification as a Nurse Practitioner or Physician Assistant with Prescriptive Authority.
  • Current RN State License.
  • Active Drug Enforcement Administration (DEA) registration.
  • Eligibility for enrollment to provide services to Medicare and Medi-Cal participants.
  • A minimum of two years of experience working with the senior demographic, with a solid understanding of chronic care and geriatric best practices.
  • Reliable transportation for travel to various locations.

Compensation: The salary range for this position is $145,000 - $174,478 per year, plus bonuses and equity. The total rewards package includes a 401(k) match, healthcare coverage, and a variety of other benefits. Actual compensation will be determined based on experience and qualifications.
