Nurse Case Manager

Nascentia Health is seeking a highly skilled Registered Nurse to join our team as a Case Manager. In this role, you will be responsible for providing direct patient care, coordinating overall home care services, and documenting care for agency clients in accordance with Quality Standards.
Key Responsibilities- Ensure quality and safe delivery of home health services, including case managing client caseloads and developing plans of care based on client assessment and OASIS evaluation.
- Complete all OASIS evaluations and ensure that documentation meets quality standards.
- Coordinate with LPNs, provide guidance and education on client care, and work in conjunction with the Clinical Supervisor and Patient Care Coordinator to efficiently manage patients using a team approach.
- Supervise paraprofessional staff relevant to patient care, communicate with the home health aide department, and maintain patient/employee confidentiality.
- Comply with all Nascentia Health policies and procedures, maintain clinical knowledge, and adhere to organizational quality improvement standards.
- Current New York State registered professional nurse license (degree must be from an accredited school of nursing).
- BSN a plus.
- Current and valid driver's license and auto insurance, and a vehicle which can be used for work.
- Self-directed and organized with the ability to work independently in a fast-paced environment.
- Previous home care/case management experience a plus.
- Proficient computer skills (including knowledge of electronic medical records software) with ability to learn new software.
- Exceptional oral, written, and verbal communication skills.
