As a Private Duty Nurse, you will be responsible for implementing physicians' prescribed plans of care, conducting patient assessments, and coordinating care with other healthcare professionals. You will also be responsible for administering prescribed medication, treatments, and therapies, as well as documenting care provided in our system of record. In addition to your clinical skills, you will be responsible for promoting health, teaching, and training family members, and following up with physicians, clients, family members, staff, and case managers on client problems, concerns, and changes in client condition. Responsibilities:

* Implement physicians' prescribed plans of care
* Conduct patient assessments and coordinate care with other healthcare professionals
* Administer prescribed medication, treatments, and therapies
* Document care provided in system of record
* Promote health, teach, and train family members
* Follow up with physicians, clients, family members, staff, and case managers on client problems, concerns, and changes in client condition

Qualifications:

* Must have and maintain an active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice
* Current CPR certification from American Red Cross, American Heart Association, or BLS
* TB skin test (current within last 12 months)
* Six months prior hands-on nursing experience preferred but not required
* Must have reliable transportation
