About the Role

As a Nurse Practitioner with Optum Home & Community Care, you will play a vital role in delivering comprehensive care to patients in the comfort of their own homes. Our HouseCalls product is designed to provide annual clinical assessments, identifying and reducing health risks, while also coordinating follow-up care to improve overall health and well-being.

Key Responsibilities:
  • Conduct thorough in-home evaluations, including reviews of past medical history, systems assessments, medication reconciliation, vital signs checks, and comprehensive physical examinations.
  • Perform evidence-based practice screenings, including point-of-care testing where applicable.
  • Identify diagnoses for care management and active medical intervention to promote treatment.
  • Determine current and past medical conditions using clinical knowledge and judgment, as well as assessment findings.
  • Communicate your assessment results to inform members' primary care physicians of potential gaps in care.
  • Recognize urgent and emergent situations and respond appropriately.
  • Educate patients on topics such as disease processes, medications, and adherence.
Requirements:
  • Active NP license or ability to obtain by start date.
  • Licensure must be unencumbered, free from any open/unresolved disciplinary actions, including probation or restrictions against privilege to practice.
  • Active national NP or PA certification or the ability to obtain national certification in the state of assignment by start date.
  • Able to meet the physical demands of the job, including lifting a 30-pound bag in and out of the vehicle, navigating stairs and various dwelling conditions, and sitting, standing, and kneeling to perform physical assessments.
  • Reliable transportation to complete home visit assessments.
Preferred Qualifications:
  • May be requested to obtain additional licensure in other geographic areas.
  • At least 1 year of clinical experience in the highest level of education, clinical setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home healthcare or home visit experience.
Compensation Information:

$120,000 - $150,000 per year, depending on experience and qualifications, plus a $5,000 sign-on bonus for external candidates.
