About the Role

We are seeking a skilled and compassionate Home Health LPN Intake Specialist to join our team at CommonSpirit Health Mountain Region. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and ensuring a seamless transition from hospital to home.

Responsibilities
  • Provide exceptional customer service to patients and families, ensuring a positive and supportive experience.
  • Participate in the discharge planning process, facilitating communication and collaboration between healthcare teams and patients/families.
  • Coordinate the delivery of agency services, ensuring accurate demographic information and appropriate physician orders.
  • Assume responsibility and accountability for facilitating, communicating, and collaborating with healthcare teams and patients/families to ensure an outstanding customer experience.
  • Promote the optimal health, well-being, and safety of patients through the use of the nursing process and in accordance with patient care standards, guidelines, and the State Nurse Practice Act.
Qualifications
  • High School Diploma/G.E.D (Required)
  • Graduate of an accredited Licensed Practical Nursing program (Required)
  • Two years of recent LPN experience
  • One year of home care, hospice, or home infusion experience preferred
  • Previous experience in discharge planning, preferred
  • Requires knowledge in nursing, evidence-based practice methods, and person-centered care concepts
  • Current state LPN license for applicable state of employment or current LPN license from a participating state in National Compact Licensure
  • Current Cardiopulmonary Resuscitation (CPR) required
  • Drivers License
  • LPN IV certification preferred
  • Current Basic Life Support (BLS) certification from American Heart Association, preferred
  • Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly
