Key Responsibilities- Administer skilled nursing care to patients in their homes, adhering to physician orders and the Plan of Care.
- Conduct ongoing assessments to identify patients' physical, psychosocial, and environmental needs, and document findings in patient medical records.
- Communicate effectively with patients, caregivers, and healthcare team members to ensure seamless care coordination.
- Provide education and support to patients and caregivers on disease management, treatment regimens, and medication administration.
- Basic Cardiac Life Support certification within 30 days of employment start date.
- Licensed Practical/Vocational Nurse (LPN/LVN) licensure.
- Valid driver's license.
- Vocational school graduate.
