Under the guidance and oversight of the Nurse Manager, the Licensed Practical Nurse (LPN) plays a crucial role in facilitating patient care services, adhering to the regulations set forth by the Florida Nurse Practice Act. The LPN is responsible for delivering patient care in alignment with the policies and procedures of Community Hospice & Palliative Care.

Key Responsibilities:

  • Uphold established productivity standards within the inpatient unit.
  • Assist in the documentation and upkeep of patient medical records.
  • Support the Interdisciplinary Group (IDG) Registered Nurse (RN) in formulating and executing the patient's Plan of Care, suggesting modifications as necessary.
  • Collaborate with the IDG Primary RN and/or physician in executing specialized tasks and procedures, including medication administration as appropriate.
  • Adopt a personalized, outcome-focused approach to patient care delivery.
  • Communicate observed changes in patients' conditions to the IDG Primary RN and physician, providing written summaries as required.
  • Guide patients and caregivers in mastering essential self-care techniques.
  • Engage in discharge planning activities.
  • Participate in IDG meetings, working alongside team members to identify and address patient, family, and caregiver needs.
  • Maintain the highest standards of professionalism, ensuring confidentiality and privacy in all aspects of patient care, in compliance with HIPAA regulations.
  • Process physician orders and accurately document verbal orders for further action, with verbal medication orders confirmed by RN signature.
  • Manage and procure all medical supplies for the inpatient unit.
  • Create and oversee staffing schedules.
  • Facilitate communication with attending physicians and primary hospice physicians during critical transitions, including patient transfers.
  • Coordinate logistics with funeral homes and ambulance services for patient transportation.
  • Prepare equipment and materials for treatment, adhering to aseptic techniques as necessary.
  • Contribute to disaster preparedness efforts within the inpatient unit and fulfill assigned roles during emergencies.
  • Engage in the limited administration of intravenous therapy (IV) upon obtaining the necessary IV Certification.
  • Participate in Performance Improvement initiatives as applicable.
  • Serve as a role model in maintaining confidentiality, professionalism, appropriate attire, and positive work attitudes.
  • Promote and embody the mission and values of Community Hospice.
  • Adhere to general inpatient and life safety standards.

Qualifications:

  • Current License as a Licensed Practical Nurse/Licensed Vocational Nurse in the state of Florida.
  • IV Certification in the state of Florida.
  • Current CPR Certification.
